Jotaro's Disc is needed to obtain Made In Heaven, as guided by Pucci.
Jotaro's Disc is actually one of the two items needed to obtain Made In Heaven, as DEO's Bone is the other item needed.
How to Obtain[]
Jotaro's Disc is an item you can obtain after defeating JOE (Jotaro Part 6). There is a small percent chance of it actually dropping, so it will be pretty hard for you to complete the Journey. (Especially when Joe Kujoe is a hard NPC to defeat)
Note that it might be easier to have a group of people to help you defeat Joe, as he does lots of damage in just a few seconds, and has a lot of health.
